This includes reports of sexual misconduct, including sexual harassment and sexual violence. Our office also handles reports of relationship violence and stalking.

Once you submit this form, you may be contacted by someone from the Equal Opportunity and Compliance Office if additional information is needed. Regardless of the nature of your report, the University will ensure that your report is forwarded to the office with responsibility for investigating and addressing it, as appropriate, based on the information you have provided.

Please note that reporting an incident without disclosing your name (i.e., anonymously) will prevent the University from being able to provide you with interim protective measures (such as academic accommodations, no-contact orders, safe housing options, change of work schedule) because University staff will not be able to contact you. Reporting anonymously may also impact the University’s ability to investigate and/or take disciplinary action.
